Hello New or Former Volunteer,
Below you will find links to the required paperwork in order to volunteer your services at the Miami Mountain Fire Lookout. The forms are in Adobe .PDF format and require Adobe Acrobat Reader to view and print them out so they may be completed. If you do not have Acrobat Reader then it may be downloaded and installed from the Adobe web site at http://get.adobe.com/reader/
On the first form headed "Agreement for Individual Voluntary Services" each person must read and then complete the following lines printing the information requested on line 1. & 2., signing and dating lines 6. & 7., and lines 8. & 9. if applicable. You must complete one of these forms for each person that is volunteering for the tower.
On the second form headed "Job Hazard Analysis (JHA)" each person must read the form completely so you understand the Tasks, Hazards and Abatement actions that can be done to reduce the risks of hazard and injury at the tower. After you have read the forms please sign and date under the section labeled "JHA and Emergency Evacuation Procedures Acknowledgment". Each person volunteering must sign and date this one form in the spaces provided.

After the forms have been received by Paul Waddell, and approved by the District Ranger, Paul will let me know and then we can schedule you for training at the lookout with one of the trainers.
